Players must navigate through numerous obstacles including cars, pedestrians, and animals while aiming to deliver newspapers to the subscribers' mailboxes or doorsteps for maximum points. Non-subscribers’ homes can be vandalized by throwing papers to break windows which gains points too but could attract negative repercussions from the residents.
Improvements in "Paperboy 2" include the ability to choose between a male or female paper carrier, each with a unique set of voices and graphics, adding a fresh aesthetic flair to the gameplay. The Sega Game Gear version, specifically adapted for the portable console, features scaled-down graphics to match the device’s capabilities but maintains a smooth gameplay experience characteristic of the Game Gear’s color screen and sound capabilities.
